Rotherham Social Prescribing Service invites grant funding applications

Rotherham Social Prescribing Service invites grant funding applications from voluntary sector organisations and groups that can provide services to improve the health and wellbeing of patients with long term conditions.

PLEASE READ THE ACCOMPANYING INVITATION TO APPLY  AND APPLICATION GUIDANCE NOTES BEFORE DECIDING TO COMPLETE AN APPLICATION FORM. THE INVITATION LETTER SETS OUT THE PATIENT OUTCOMES AND EVALUATION CRITERIA ON WHICH YOUR APPLICATION WILL BE ASSESSED.

  • Only not-for-profit organisations from the voluntary and community sector that provide services in Rotherham are eligible to apply for funding
  • Grants will only be awarded to organisations to deliver services or activities for patients with long term conditions referred by the Social Prescribing Service.
  • Organisations or groups applying for £5,000 or more are required to complete an initial Pre-Qualification Questionnaire (PQQ) before an application for grant funding will be considered (there are two versions of the PQQ – a passport version and a full version and a cover letter– please contact the Social Prescribing Service for details if you have not already completed a PQQ). Grant applications of less than £5,000 do not require completion of a PQQ.

If your organisation or group would like to apply for a large funding grant (£5,000 or more) or a small grant under £5,000, you should complete and return a signed large or small grant application form and return to VAR’s Social Prescribing Service team before either of the two deadline dates:

Friday 19th October 5pm

Friday 2nd November 5pm

Applications will be assessed by a multi-disciplinary funding panel soon after each closing date and you will hear of the decision on your application by 31st October if meeting the first deadline or 16th November if meeting the second deadline
